- The online form is to be completed by a participant requesting CEU credits for a previously attended workshop or training.
- The Participant form must be submitted no later than two weeks after the completion of the program.
- Once your digital participant form and payment are received you’ll receive an email with a digital CEU certificate within 3-5 business days.
